TruePath Medical Supply, LLC ("TruePath," "we," "our," or "us") is committed to protecting the privacy of our patients and website visitors.   • Insurance information (plan name, member ID, group number)
  • Prescription and physician information
  • IP address (collected during patient packet acknowledgements for compliance purposes)
  • Browser type, operating system, and referring URLs (collected automatically)

Protected Health Information (PHI)

We collect PHI as necessary to provide durable medical equipment and related services, including diagnosis codes, physician orders, insurance claims data, and delivery records.

3. How We Use Your Information

We use your information to: (a) provide and bill for DME equipment and services; (b) verify insurance eligibility and obtain prior authorizations; (c) communicate with your healthcare providers; (d) comply with legal and regulatory requirements; (e) improve our services and website; and (f) send appointment reminders and service notifications.

4. HIPAA Notice of Privacy Practices

As a HIPAA-covered entity, TruePath Medical Supply maintains the privacy of your Protected Health Information and provides you with notice of our legal duties and privacy practices. We are required by law to maintain the privacy of PHI, provide this notice, and follow the terms of this notice.

  • Right to Access: You may request a copy of your health records.
  • Right to Amend: You may request corrections to your health information.
  • Right to Restriction: You may request that we restrict certain uses of your PHI.
  • Right to Accounting: You may request a list of disclosures of your PHI.
  • Right to Confidential Communications: You may request communications at a specific location or phone number.
  • Right to Complain: You may file a complaint with us or the U.S. Department of Health and Human Services (HHS) if you believe your privacy rights have been violated.

5. Information Sharing

We do not sell your personal information or PHI to third parties. We may share your information with: (a) your treating physicians and healthcare team; (b) your insurance carrier for claims processing; (c) our suppliers (DDP Medical, GEMCO Medical) as necessary to fulfill your equipment order; (d) government agencies as required by law; and (e) service providers who help us operate our business under strict confidentiality agreements.

6. Electronic Acknowledgements & IP Logging

When you submit the Patient Information Packet acknowledgement form on our website, we record your IP address as part of the electronic acknowledgement. This is required for Medicare and AHCCCS compliance documentation purposes and serves as a legally recognized electronic signature of receipt. This data is stored securely and is not used for marketing purposes.

7. Cookies & Website Data

Our website uses cookies and similar technologies to improve your experience, analyze site traffic, and provide relevant content. You can control cookies through your browser settings. We do not use cookies to collect PHI.

8. Data Security

We implement administrative, physical, and technical safeguards to protect your information, including encrypted data transmission (HTTPS), secure server infrastructure, access controls, and regular security reviews consistent with HIPAA Security Rule requirements.

9. Data Retention

We retain patient records for a minimum of 7 years from the date of service, or as required by applicable state and federal law. Electronic acknowledgement records are retained indefinitely as compliance documentation.
